1. To begin the Perkins Experience survey, go to either www.perkinsfeedback.com or www.perkinsexperiencesurvey.com. Both websites bring you to the Perkins Experience survey questionnaire.
2. Next, you will be prompted to enter a survey code. This code consists out of 15 digits. You can find it on the receipt you got from your visit at a Perkins restaurant. To be eligible, it must have been issued within three days prior of completing the survey.
3. If your coupon doesn’t have the code shown in the image, you can access an alternative link from that page which will have you complete different information. This will include your order number, store number (this is the Perkins restaurant number) and the time and date of your visit. This information is contained on your receipt.
