- Fill in Five Below store address. Before you can enter the survey, you must choose the state in which you visited Five Below. After that, you can select the address of the Five Below store from the drop down menu.
- Enter email address. Once you have selected the Five Below store you visited, you will have to write your email address.
- Answer the FiveBelowSurvey questions. You will have to answer all the question in order to enter the Five Below Sweepstakes. Once you have finished taking the FiveBelowSurvey, you will automatically enter the sweepstakes.
